Ø Summary of duties/responsibilities while employed.
Ø Email address of supervisor(s).
Ø In order to complete your administrative application, CCSD requires that we contact the two most recent principals who have supervised your performance. If you are currently a principal or central office administrator, please list your two most recent supervisors. Please list below their names and current contact information. It is your responsibility to obtain current contact information.

Ø Educational information (name of college, degree earned, dates attended).
Ø Personal email address is highly recommended to enter on your online application to receive future correspondence and updates from Cobb County School District HR Department. If you do not have an email address, email addresses are available for free through www.yahoo.com or www.hotmail.com.
Ø Detailed information if you have ever been convicted by federal, state or other law enforcement authorities or pleaded nolo contendere for violation of any federal law, state law, county or municipal law, regulation or ordinance. You must include any offense for which a fine of $200 or more was imposed. (Do not include any offense that occurred before your eighteenth birthday.)
