GoalView State Data Checklist

 

Requirement: Verify and update State Data for the December 1 count.

1. Verify your caseload as of December 1

 

  • Are all active students listed on the Goalview caseload (Views) report?

 

 

 

  • Have any new students been enrolled?  Were they flagged on School Max for Goalview?  Do they appear on Goalview yet?

 

 

 

  • Have all inactive students been withdrawn (from specific service[s]) and/or exited (from special education program) on Goalview?

 

 

 

  • Are any students listed who have not been staffed in to special education yet? (These should not be counted for December 1!)

 

 

 

2. Check each student’s Hours Per Week

 

  • Are the Hours Per Week accurate for each active service? (See *below)

 

 

 

  • Do any students have more than 30 total hours of special education indicated?  (Please verify that hours codes for all services are correct)

 

 

 

  • Are Inclusion segments (from Additional Services screen) included in your calculation of the total Hours Per Week for that (Primary) service?

 

 

 

3. Check each student’s Program Summary

 

  • Do any of the first 12 services listed under Subject/Service have Hours provided? (Please enter under State Time Code)

 

 

 

  • Have you entered the same time code as is listed on Active Exceptionalities for the following services = adapted p.e., audiology, occupational therapy, physical therapy, school health (nursing), and orientation & mobility?

 

 

 

  • Did you enter a time code for any of the following that are ONGOING (not one-time) services through the IEP = counseling, diagnostic, psychological, recreation, or school social work?

 

 

 

  • Did you enter a time code for Transportation, Special if any of the following conditions apply = adapted equipment on bus, bus aide assigned to student, or adapted route outside district boundaries?

 

 

 

 

*Common Hours Per Week & State Time Codes for special education services:

1 segment/day = 5                           1 block High School segment/day = 6-10

2 segments/day = 6-10                   2 block High School segments/day = 11-15

3 segments/day = 11-15                 3 block High School segments/day = 21-25

4 segments/day = 16-20                4 block High School segments/day = over 25

5 segments/day = 21-25                

6 segments/day = over 25
