______Wheeler Contract signed by:  Parent & Student              

                                                                                   

______Truancy form (JLIE-1) signed by:  Parent & Student

                                                                                   

______Copy of student’s current Georgia driver’s license.

 

______  Copy of Student’s proof of insurance valid when submitting the documents, December 6, 2012.  This document must reflect the company’s name, telephone #, policy #, dates of coverage, and the vehicle the student will be driving.

 

_____ $50.00 check made out to Wheeler High School.

 

______ Do not staple the application, use a paper clip.  Please put the papers in the order listed above with the check paper clipped on top.

 

______Make sure that all information requested on the application is entered, for example: VIN #, Tag #, License #, School Site, etc.          

 

______ Applications will be not be processed if you have an unpaid parking ticket or owe for textbooks.
