This form can ONLY be completed by a CCSD guidance counselor, magnet coordinator, or school administrator.  The form is for the purpose of approving student enrollment in online courses delivered by Cobb's eHigh School.  Students approved using this form are enrolling in an eHigh School course as an: 

1.   FTE student who will take the course as part of the regular school day (including 5th Year Seniors)
