1. When do my benefits become effective and/or end?
Your benefits become effective the first day of the calendar
month after you complete one full calendar month of
employment. Benefits are prepaid one month in advance from a
payroll deduction. Benefits continue one month following the
last payroll check from which insurance deductions are taken.
However, if you work even one day in a month you are eligible
for a deduction that month and your benefits will continue
until the end of the following month.
2. What do I do if I lose my insurance ID card?
Indemnity Option and PPO and PPO Choice (Preferred Provider
Option) identification cards are provided directly from the
Georgia Department of Community Health. The UnitedHealthcare
of GA, Blue Choice, Kaiser Permanente and CIGNA Healthcare of
GA HMO (Health Maintenance Organization) and CCO (Consumer
Choice Option) cards are generated by the appropriate
insurance companies. Both the CIGNA Dental PPO Plan and the
Metlife Low Dental Plan provide identification cards to
enrollees. If you lose your
card please contact your provider. To contact the appropriate
provider the telephone numbers are located on the
CCSD web
site and on the inside back cover
of your Benefits handbook.
3. Where can I get detailed information regarding my benefits,
claims, pre-existing conditions and out-of-state coverage?
Review your State Health Benefit Plan Updater, Decision Guide,
HMO Member Handbook or the Summary Plan Description provided
by the insurance company. If you need further information,
please contact the appropriate insurance company. Please refer
to your Benefits handbook and/or ID card for telephone numbers
and/or web-site addresses.
4. When do I apply for a leave?
Once you have been out of work for 10 consecutive days for
your own illness or 5 consecutive days for a family member’s
illness, you must have your leave time approved through the
CCSD Benefits Office. It does not matter how many short term
leave days you have available in your short-term leave account
you must still have your leave time approved.

5. When will deductions and reimbursements for my Optional
Spending Account expenses occur?
The Plan covers expenses incurred between January 1 and December
31. The deductions are taken from Employees paycheck January
through December. You are eligible for reimbursement once the
first deduction in January occurs.
