Clinic Information

In Cobb we take the education of our students very seriously and we know that HEALTHY children learn best!  In order to keep your child healthy and safe at school, we provide a licensed nurse in every school building!   We invite you to contact your school nurse to discuss any health needs or concerns for your child.  While preparing for Kindergarten please also obtain documentation from your health care provider regarding ALLERGIES, ASTHMA, DIABETES, SEIZURES or other health conditions that may require our assistance during the school day. Documentation paperwork can be obtained on Cobb County School District website, then given to your doctor to complete. Links are provided below:

In order to have your child placed in a classroom and have your registration process complete, all of the following items must be received by the school.

* 2 Proofs of Residency for Mableton District

* Immunization Form   GA-3231

* Birth Certificate with raised seal

* Hearing/Vision/Dental/Nutrition Form GA-3300

* Social Security Card

* Copy of Report Card

* Withdrawal Form

If you participate in the morning carpool, this letter is for you.  We need your help!  Please help us keep your children safe by following ALL Carpool and Parking Lot procedures.  Unfortunately, we have had several recent close-calls involving inattentive drives and unsupervised children walking through the parking lot and across the streets.  Your efforts to create a safe environment for all of our students, your children, is greatly appreciated.  Thank you for your patience and cooperation regarding the carpool and parking lot procedures.

Si usted viene a dejar a sus hijos por la mañana, esta carta es para usted. ¡Necesitamos su ayuda! Por favor ayúdenos a mantener seguros a sus hijos siguiendo TODAS las reglas y procedimientos de tráfico y estacionamiento.  Por desgracia, hemos tenido varias faltas y casi accidentes por falta de atención de los padres e hijos caminando sin supervisión entre los coches en el estacionamiento y calles. Sus esfuerzos para crear un ambiente seguro para todos nuestros estudiantes, sus hijos, es muy apreciada. Gracias por su paciencia y cooperación en relación con los procedimientos de tráfico y estacionamiento.



 ·        Please allow your child to walk himself/herself to class.  If you have a need to walk to the classroom before 7:50; however, you will need to sign in at the front lobby window and receive a yellow visitor sticker.  School begins at 7:50; therefore, visitors are only permitted to classrooms between 7:15 and 7:50.

·        CCSD Policy requires that all parents/guardians show ID when picking up a child from school.  To ensure the safety and security of our students, no child will be released to an adult without an ID.

School starts at 7:50 am

Doors open at 7:15am
