Cobb Virtual Academy and Georgia Virtual School Verification Form

ONLY a CCSD guidance counselor, magnet coordinator, graduation coach or school administrator may complete this form.

 

The form is for the purpose of approving FTE enrollment (including 5th year seniors) in online courses delivered by Cobb Virtual Academy and verifying ALL CCSD students (tuition and FTE) enrolling with the Georgia Virtual School.

 

Counselor Responsibilities :

1.  Complete this form.  Note that * means the information is required. 

2.  Print the form for your records.

3.  Click the Submit button at the bottom of the page.

Complete the registration process with the correct program (Cobb Virtual Academy  www.cobbvirtualacademy.org  or Georgia Virtual School www.gavirtualschool.org during the registration period.
