Please contact the Counseling Office for more information and to schedule an appointment for registration.

To register a new student at Durham Middle School, you will need the following:

· Certificate of Immunization (GA Form 3231): Students coming from an out-of-state school or those who have been absent for more than 12 months must obtain a Georgia Form 3231 from the Health Department. This is required to be admitted to any facility or school in Georgia for the first time on or after August 1, 2000.

· MMR for students entering the 6^th grade: All students entering the sixth grade must have had two 2 doses of Measles vaccine, 2 doses of Mumps vaccine and the Rubella vaccine. Proof must be provided to the middle school on a Georgia Health Form.

· Varicella for students entering the 6^th grade: All students entering the 6^th grade must show proof that they have had 2 doses of the Varicella immunization, or have documentation showing that they have had Chicken Pox (GA form 3189).

· Hepatitis B Vaccine is now required for a child at the time of entry to school on or after August 1, 2000. This new requirement does not apply to students already enrolled in a Georgia school.

· Certificate of Ear, Eye and Dental Exam (GA Form 3300): Students coming from an out-of-state school must obtain a GA Form 3300 from the health department. The cost for Vision screening is $10.00, Hearing screening is $10.00, and Dental screening is $4.00. Until July 31^st all screenings are available from 1:00 PM – 4:00 PM. Beginning August 1, screenings are available all day.

· Custody or Guardianship: If custody or guardianship papers are applicable, you must show proof of custody at the time of registration.

· TwoProofs of Residency: Any documentation showing your name and properly address is needed is for proof that you reside in our attendance area. Some examples are: contract on a home, deposit on utilities, lease agreement, etc. If you will not be occupying the residence when school begins, you must complete a 30 day extension form.

· Your child's Social Security Number or you may complete a waiver (available at the time of registration).

· Current Grades and Test Scores: The last report card will be sufficient. Current standardized testing results will help us in placing your child in academic classes.

· Complete address including zip code of last school: We will request records with this information. Please do not have the information sent to the school.

· Special Education Students: A current IEP.

· Gifted Education Students: A current Gifted Education placement form. Review of records using Georgia's guidelines will be used to determine your child's eligibility.
