MEDICAL INFORMATION SHEET
Hello! Did you know that physical education is one of the most
important subjects your child can learn in school?
By gaining early success in the basic physical skills and concepts, your
child will feel better about his/her ability to participate in recreational
activities; be more successful if they choose to compete in youth sports
leagues; and find it easier to accept themselves and be accepted by their peers
as they move throughout their school and adult years. Their success in physical education impacts their success in
school!
In addition, learning basic movement skills and knowledge in
physical education helps your child feel good about moving, which in turn
encourages them to be active and healthy. And
from our country’s Surgeon General, we know that physical inactivity is one of
the leading causes of heart disease and mortality in our country.
So you see, good physical education CAN make a difference!
In physical education class this year, a student will learn
and continue to refine skills and concepts centered around the following themes:

AWARENESS
(body parts and body shapes)

SPATIAL
AWARENESS (moving
through space by self and with others; moving in different directions; at
different levels; and different pathways)
ü
EFFORT
(moving with variations in force, speed, and smoothness/control)

(with self and objects; to partners; to groups)
ü
LOCOMOTOR
MOVEMENTS (walk, run, hop, skip, jump, gallop, slide, leap)
ü

& LANDING (with jump ropes; from
different heights, for distance)

BALANCING, CHASING, FLEEING, DODGING
(skills used in tag and other group games

DRIBBLING
WITH THE HANDS

Activities in the above are put into the contexts of
appropriate games, dance, rhythms, and educational gymnastics.
They are sequenced with your child’s abilities in mind, with the end
result to help your child learn and be successful in these situations.
Please
be aware of the following guidelines for physical education
1.
If your child needs to be
excused from physical education
activities, please write a note to us and have your child bring it to us during
PE class. Parental notes can only
excuse for up to 3 days; anything requiring a longer time period will need a note
from a doctor. Every child
will be expected to participate in the activities unless excused by a note.
2.
If a doctor limits physical education participation,
please ask him/her to be specific as to the amount of activity or what your
child can/cannot do, as well as when he/she is able to fully participate in
class.
3.
For your child’s safety as well as the liability involved,
your child is required to wear proper footwear in physical education.
Proper footwear includes tennis-type/sneaker shoes—those with a flat
bottom (no heel) and the foot fully covered.
Clogs, sandals, dress shoes, boot-like shoes, opened-back (no heel)
tennis shoe, and any heeled shoes are NOT appropriate types of footwear for
physical education class. Your
child’s participation in class may be limited, depending on what we are doing
that day. If your daughter wears a dress or skirt, please have her wear
shorts underneath. Also, socks are
required due to activities in which we take our shoes off.
Please fill out the form attached and return to us this week.
Students may not participate in vigorous activities until this note is
signed and returned. Thanks for all
your help, and we look forward to helping your child learn this year.

Demonstrates
competency in many movement forms and proficiency in a few movement forms:
The
intent of this standard is the development of movement competence and
proficiency. Movement competence
implies the development of sufficient ability to enjoy participation in physical
activities and establishes a foundation to facilitate continued motor skill
acquisition and increased ability to engage in appropriate motor patterns in
daily physical activities.
Applies
movement concepts and principles to the learning and development of motor
skills: This
standard concerns the ability of the learner to use cognitive information to
understand and enhance motor skill acquisition and performance.
This includes the application of concepts from disciplines such as motor
learning and development, sport psychology and sociology, biomechanics, and
exercise physiology.
Exhibits
a physically active lifestyle: The intent of this standard is to establish
patterns of regular participation in meaningful physical activity.
This standard should connect what is done in the physical education class
with the lives of students outside of physical education.
While participation within the physical education class is important,
what the student does outside the physical education class is critical to
developing an active, healthy lifestyle. Students
are more likely to participate if they have had opportunities to develop
interests that are personally meaningful to them.
Exhibits
a physically active lifestyle: The intent of this standard is for the student
to achieve a health-enhancing level of physical fitness.
Students will be encouraged to develop higher levels of basic fitness and
physical competence as needed for many work situations and active leisure
participation. Health-related
fitness components include cardio respiratory endurance, muscular strength and
endurance, flexibility, and body composition.
Demonstrates
responsible personal and social behavior in physical activity settings:
The
intent of this standard is achievement of self-initiated behaviors that promote
personal and group success in activity settings.
These include safe practices, adherence to rules and procedures,
etiquette, cooperation and teamwork, ethical behavior in sport, and positive
social interactions.
Demonstrates
understanding and respect for differences among people in physical activity
settings: The intent of this standard is to develop
respect for individual similarities and differences through positive interaction
among participants in physical activity.
Understands
that physical activity provides opportunities for enjoyment, challenge,
self-expression, and social interaction: This
standard is designed to develop an awareness of the intrinsic values and beliefs
of participation in physical activity that provides personal meaning. Physical activity can provide opportunity for self-expression
and social interaction and can be enjoyable, challenging, and fun.
These benefits entice people to continue participation in activity
throughout the life span.
Physical
Education grades will be based on participation and conduct and will reflect
qualities discussed within these standards.
