HILLGROVE HIGH SCHOOL
New Student* Registration

enrollment appointment.
1. PARENT OR GUARDIAN MUST BE PRESENT.
A. If the student lives with only one natural
parent, Hillgrove must be provided
with the appropriate legal
documentation showing that the enrolling parent
was designated by the court as the
custodial parent. If the student is now
living with the non-custodial parent,
a notarized statement from that
custodial parent giving permission
for the child to live with the non-custodial
parent and to attend Hillgrove High
School is required.
B. If the student is living with someone other than a natural
parent and is under
18 years of age, that individual must
call 770-426-3341 for special enrolling
instructions from Cobb County
Schools. They will also be required to file for
legal guardianship of the enrolling
student through the Cobb County Probate
Court, 32 Waddell Street, Building D,
Marietta (770-528-1900).
C. Any custody circumstance not covered under section 1
should be reviewed
with a guidance office staff member
to receive appropriate instructions
concerning forms and/or documentation
needed.
2. TWO DOCUMENTS SHOWING PROOF OF RESIDENCY:
These must show that the parent(s) or guardian(s) with whom the
child lives is living in the Hillgrove district. This could include
a lease or contract for a home or apartment, plus a utility bill
such as water, electric, natural gas, or telephone that shows both
name and address.
If the student and his parent(s) or guardian(s) are
living with another adult and/or family at a residence other than
their own, then residency must be documented by the owning/leasing
party. They must appear in person at Hillgrove with a lease or deed
for their home or apartment, plus a bill that shows their name and
address from one of the following utilities: water, electric,
natural gas, or telephone. While at Hillgrove, they will sign
appropriate documentation of the student’s residency status.
3. OFFICIAL WITHDRAWAL PAPERS AND CURRENT
WITHDRAWAL GRADES:
These are required from the last school attended, unless the student
leaves the previous school after the end of the entire school year.
Placement is provisional until the student’s official transcript has
been received to verify grade levels and course credits. Eligibility
for competitive activities is determined by the student’s official
transcript, which must be received before participation. If the
student has completed the 8th grade, but hasn’t started 9th grade,
documentation of promotion to 9th grade is required.
4. UNOFFICIAL TRANSCRIPT: Transcripts are
needed from any and all former high schools; all courses taken,
grades and credits must appear on transcripts. Complete and current
addresses for those schools are also needed.
5. COPIES OF STANDARDIZED TEST SCORES.
6. COPY OF DISCIPLINE RECORD: A discipline
record from the former school must be provided, or you may sign a
release for Hillgrove to obtain this record.
7. SOCIAL SECURITY NUMBER: An official copy of
the student’s Social Security card/number or parent waiver.
8. HEALTH FORMS: Both the Georgia Certificate
of Immunization (Form 3231) and the Georgia Ear, Eye, and Dental
Exam Form (Form 3300) for your student are needed PRIOR to
enrollment.
A. If you have come to Georgia
from another Georgia public school, your former
school should have included these
certificates in the permanent file and should
have supplied you with a copy in the
withdrawal forms to give to the next
school.
B. If you are coming to Hillgrove from any other school, you
will need to take your
student and all former immunization
records (from private physicians, schools
and any other sources) to one of the
two local Cobb County Health
Departments. They will review your
health records, give your child and
necessary immunizations, and then
provide you with an appropriate form.
While at the Health Department, your student must
also get a brief dental exam in order to issue the Georgia Dental
Exam Form #3300. Do not bring any former dental records to the
Health Department, as they cannot be used.

9. PRIVATE SCHOOL/HOME SCHOOLING: If you are
coming from a private school or home schooling, a statement of
accreditation must be provided at registration. Students entering
from a private or home school may be required to validate a passing
grade by the next sequential course or by exam credit.
