6th GRADE WINTER CONCERT PERMISSION FORM

Dear 6th Grade Band Parents,

As you can see on our  band calendar, our winter concert date is quickly approaching. We need your signature on this final permission form so that we may begin our final plans for this performance. The concert is on Tuesday, December  16, 2008, at 7:00 PM.

6:30 PM Students arrive at the Dodgen Band Room dressed in the full band uniform to begin the warm-up session
7:00 PM Concert Begins
7:30 PM Concert Ends
7:30 -8:00 PM Reception in the Dodgen Cafeteria for students, parents, family, and friends.

Please contact your grade level parent representative at BandRep6@DodgenBand.org if you are available to assist with the reception. After filling in the information below, please sign and return the lower portion of this form no later than Monday,  November 17, 2008.  Thank you!

I understand that my child must wear the full band uniform (Band Shirt, long black pants, black socks, and dress shoes).
