Dodgen Band Ensemble Sign-Up Form

Please place a check mark in the box to indicate which ensemble you wish to join.  Students may choose to participate in more than one ensemble.  PLEASE CHECK THE BAND CALENDAR FOR ALL DATES ASSOCIATED WITH YOUR CHILD’S ENSEMBLE GROUP (S). The GMEA event  for 1^st semester participants is paid for through Band funds.  If a student signs up for first semester , then drops , that student must refund the Band account the amount paid for their participation in the GMEA event.

Performance dates during the year are posted on the band calendar found at www.DodgenBand.org. Membership in an ensemble can be for the first semester only, second semester only, or both semesters.  Membership in the Jazz Band will be for the entire school year.  It goes without saying that a student should not attempt to be in two or more groups that happen at the same time.  There are many activities each day of the week at Dodgen to choose from…make sure you do not create schedule conflicts.  Please do not sign up for a morning ensemble if you are unable to get your child to school prior to 7:55AM.  I must open the building for all ensemble members at 7:55 AM. The doors are then locked and rehearsals begin back in the band room.  I can not return to the front of the school to open doors for late students while rehearsals are in session. Oher students, teachers, or Dodgen staff members are not allowed to open the front doors for late band students.

 NOTE: Smaller groups can rehearse in the band room during the morning using our smaller practice rooms. 

Permission Form

My child has permission to participate in the ensemble(s) checked above.  I understand that membership is voluntary and that attendance needs to be regular.  I understand that after two absences for any reason and/or after being tardy three times, a student will be unprepared for performance and will be removed from the ensemble until the following semester.  This means that I understand I am not to schedule my child for two different activities that occur at the same time. I understand that I must have my child at the Dodgen main entrance no later than 7:55 AM to be admitted into the building.

I am also aware that the Dodgen Band funds will cover the participation cost for the GMEA Performance Evaluation during the first semester and that I am not to send in funds for that event. I also understand that I must refund the participation fee to the band account if my child withdraws membership in the ensemble prior to the GMEA event or if my child is removed from the ensemble due to irregular attendance and/or excessive tardiness.  (NOTE: Since groups only meet once each week, there will only be 14 rehearsals prior to each performance. Students are encouraged to schedule their academic help sessions with other teachers on days not involving their rehearsal. Thank you!)
