New to Simpson School District

(This does not include 5th graders from one of Simpson’s feeder elementary schools.)

Go to Simpson Student Registration and Enrollment.

The following documents are required for enrollment in Cobb County Schools:     

1. Certified Birth Certificate (not a hospital certificate)     

2. Two Proofs of Residency (lease, phone bill, utility bill, current bank statement, etc.)     

3. Form 3231: Current Immunization (marked complete for school attendance)    

4. Form 3300: Vision, Hearing, and Dental Screening (dated within 12 months of first day of school)  

 5. Social Security Card (or waiver)

6.  Last Report Card

7.  Any standardized testing scores

8.  Any Gifted Paperwork (if applicable)

 Forms #3231 and #3300 are available from physicians or the Cobb County Health Department. Form #3300 - Certificate must be dated within 12 months of the first day of school.
