• GA Form 3231/Immunization
• GA Form 3300/Dental, Hearing, Vision
• Birth Certificate
• Copy of Social Security Card or Waiver
• Two Proofs of Residence

It would help us as well if you could provide the following: 

1. Report Card from last school year/marked promoted

2. Copy of any testing from last year/ ITBS, CRCT
