Summer New Student Registration
Also other grade levels will be registered for next year.
Tuesday, July 22, 9 a.m.- 6 p.m.
Kindergarten
Student must be 5 years old on or before September 1, 2007.
Effective July 1, 2007, Children entering Kindergarten must
have 2 doses of Measles vaccine,
2 doses of Mumps vaccine, and 1 dose of Rubella vaccine or
Proof of Immunity against Measles, Mumps and Rubella.
First Grade
Student must be 6 years old on or before September 1, 2007.

to register a child for school:
1. Two proofs of
Residency (Apartment lease, phone bill, utility bill, etc.)
2. Birth
Certificate
3. Immunization
Form 3231 (Student must have 2 doses of Varicella (Chicken
Pox) vaccine)
or documentation of this disease OR Proof of
immunity.
4. Eye, Ear &
Dental Form 3300
5. Social Security
Card
