2008-2009 ASP INFORMATION PAGE

Welcome to our After School Program. We plan to provide a fun, save environment for your child in the program during the school year. Please read carefully and make sure you understand the following rules before signing.

1. Effective summer 2004 there is a $10.00 registration fee.

2. Effective summer 2004 Program tuition is $7.00 per day.

3. Every child attending the ASP must have a current ASP Registration From on file at our school. It is the parent’s/guardian’s responsibility to keep all information current.

4. The ASP is a PREPAID program. Payments must be received prior to attendance.

5. All Cobb County School District policies in the student handbook apply to ASP and must also be followed.

6. The hours of the program are from the time school ends until 6:00 p.m. on the days school is in session. Attendance will be taken 15 minutes after school dismissal. If your child is picked up after that time, the account will be charged for the day.

7. ASP checks must be made payable to the school. Only include ASP payments whether paid by cash or check. Do not include other school money, such as lunch, PTA, or field trips. Change will not be given. Any money over the exact amount will be credited to ASP.

8. You must request ASP refunds in writing and they must be received by the last week of the school year.

9. MONTHLY OR SUMMARY STATEMENTS OF STUDENT ASP ACCOUNTS ARE AVAILABLE BY REQUEST.

10. If your bank returns a check unpaid, the check will be charged back to your child’s ASP account. If the returned check causes the ASP account to become delinquent, you will have three days from the date you were notified by the school to bring the account current. Failure to pay within this time will result in your child being withdrawn from the After School Program. If two checks are returned, only cash or money order payments will be accepted the remainder of the school year.

11. Anytime there is a change in your child’s ASP attendance, written notification is required stating the necessary changes.

12. Only people designated on the registration form will be allowed to pick up your child. This is only for protection of your child. IDENTIFICATION WILL BE REQUIRED. Your child must be signed out at the desk every day.

13. A LATE FEE of $1.00 per minute will be charged to the parent for every minute past 6:00 p.m. according to the school clock. YOUR CHILD WILL BE WITHDRAWN FROM THE PROGRAM AFTER THE THIRD LATE PICK UP.

14. To ensure the safety and security of all students in the After School Program and if an emergency happens that threatens the welfare of a child, a school official will contact the Cobb County Police Department. Late pick up by a parent/guardian may be considered an emergency.

15. The After School Program is a privilege and each child is expected to behave properly. The same behavior expectations apply in the ASP as during the regular school day. Inappropriate behavior may result in suspension or withdrawal of your child from the program.

16. If your school closes because of inclement weather or any other reason, ASP will close also. In this case, the instructions given to your child’s homeroom teacher on the inclement weather form will be followed.

17. Students may be suspended or withdrawn from the After School Program for the following reasons:

· Three Late Pick-ups

· Discipline Problems

· Nonpayment of Tuition and Fees

· Principal’s discretion
