Bullard Registration Information

Registration for all NEW students entering Bullard Elementary for the FIRST time in
2008 - 2009 that live in the Bullard Elementary School district will be conducted at Bullard Elementary.  If your child is currently enrolled in Kindergarten at Bullard Elementary, you do NOT need to register for first grade.

Registration Date:  April 2, 2008

Registration Time:  8:30 - 11:00

Bring the following documents and information to registration:

• Certificate of Immunization (form 3231)

• Eye, Ear, and Dental Form (form #3300)

• Original Certified Birth Certificate from the State Bureau of Vital Statistics

• Social Security Card or Waiver

• Proof of custody if child does not live with both parents

• Two proofs of residency in school attendance zone

• Copy of last report card and complete address of previous school if registering for First Grade

• Names and phone numbers of persons who may pick up your child in an emergency

• Name and phone number of your child's physician
