TRANSCRIPT/RECORDS

All requests should be made in writing at least 10 days in advance.

First copy is free.  Other copies of transcripts are $2.00 each.
bullet

Payment must be remitted in the white student payment envelopes available from the front office.

 It is our policy not to recommend or fail to recommend any student. The student transcript is representative of a student’s work.

Physician or Mental Health providers’ request for completing forms/ observations/checklists/ is considered optional. All forms must  be submitted to the Counselors office for approval from CCSD,  the forms will then be sent to the teachers to be filled out. Forms will be directly returned to the professional requesting the service. Please allow 3 - 4 weeks for this process to be completed.
