If your child needs to take
medication on an ongoing basis use
the
AUTHORIZATION TO GIVE MEDICATION
AT SCHOOL form. This includes
all prescription medicine, cough
and cold medicine (except
lozenges), antihistamines, and
over-the-counter medication not
listed below.
Middle school students may carry:
Tylenol, Advil, and their generic
forms, antacid, and throat
lozenges. The
OVER-THE-COUNTER MEDICATION
form must be on file in the clinic
and signed by parent, student, and
nurse.
