REGISTRATION INFORMATION

An appointment is required for registration.  Please read the following information prior to registering your student. Parents should provide all of the documents listed below at the registration appointment in order to not delay the process. 

PARENT MUST BE PRESENT: A biological parent or legal guardian must accompany students under age 18.

Step parents are not considered legal guardians unless they have completed the adoption process.

HEALTH FORMS:
All students entering school in the Cobb County School District shall be required to present a Georgia Certificate of Immunization (Form # 3231) in compliance with state and local regulations. (Policy JGCB)

The State Department of Education also requires that all new students must submit an Ear, Eye and Dental Certificate (Form # 3300).  These forms can be completed by the local health department (some fees may apply) or a private physician or dentist.  Please contact health departments in advance for the hours and times available to obtain forms #3231 and #3300.

PROOF OF RESIDENCY:

Two forms of proof that you and your student reside in the KMHS district must be provided at registration.  Residency can be verified by the following documentation which includes an address within Kennesaw Mountain High School’s district: a final settlement statement, complete with necessary signatures and a closing date; a lease or rental agreement including name, address, telephone number of the lessor; current utility monthly statement, current Georgia Driver’s license, current bank, credit union, or other financial institution documentation, mail delivered by the U. S. Postal Service excluding mail addressed to occupant; employer documentation, Cobb County Property Tax statement for the current year, voter registration documentation, or a current motor vehicle registration.  The student must reside full-time with the biological parent/guardian whose residence is in the Kennesaw Mountain district.

SOCIAL SECURITY CARD:
An official copy of every student’s social security number must be incorporated into his/her school records.  If you object to the use of your student’s social security number, you may elect to complete and sign a waiver.

PROOF OF DATE OF BIRTH:  Original birth certificate, passport, etc.

OFFICIAL TRANSCRIPTS and WITHDRAWAL FORMS: 
A student’s complete high school transcript must be provided at the time of registration so that appropriate courses may be selected.  Withdrawal forms are required if you are enrolling after the semester has begun.  Placement is tentative until the student’s “Official Transcript” has been received to verify grade levels and course credits.  Eligibility for competitive activities is determined by the student’s official transcript, which must be received before participation. Ninth graders entering high school for the first time need an official eighth grade report card showing proof of promotion to ninth grade.

PRIVATE SCHOOL/HOMESCHOOL:
A statement of accreditation must be provided at registration if entering from a private school.  Students from private schools or homeschool could have to validate a passing grade by passing the next sequential course.  Some courses additionally require a passing score on the Georgia End of Course Tests. (EOCT)  Rising ninth graders from non-accredited schools or homeschool may be required to take district proficiency exams.

DISCIPLINE RECORDS: 
We require a discipline record from the student’s former school or you must sign a release for Kennesaw Mountain to obtain this record.
