Registration Information

All rising 6th graders and all
new 7th and 8th graders moving from out of state must have documentation on
a GA immunization form that they have had 2 MMR (measels, mumps, rubella)
shots and also that they have had 2 Varicella Shots (chicken pox vaccine) or
proof of the disease. This must all be documented on a GA immunization form
3231. This documentation can be obtained from a local pediatrician and also
from the local health department.
