Summer Registration
for new students attending Lost Mountain in the fall will be held on
Tuesday, July 22nd from 9am to 6pm. in the Media Center.
Please contact the school for an appointment.
Per Cobb County
guidelines you will need the following to register:

An official
copy of the student’s Social Security number

Certificate of
Immunization on Georgia Form 3231
*Please note if you are transferring from out of state or
entering 6th grade, your child is required to have two
doses of MMR and two doses of Varicella.

Dental, Vision Screening Certificate on Georgia Form 3231
