Change of Transportation

Students need a note sent to their classroom teacher with any transportation changes.  If a change needs to be made after the child arrives at school a signed note must be faxed to the school office.  Transportation change notes need to include:

• date note is written
• student's first and last name
• teacher's name
• date(s) the transportation change is in effect
• specifics of the transportation change

        -name of who is picking the child up
                                   or
        -what bus number the student is riding and 
         where he/she is suppose to get off the bus
         (friend's name, grandparent's name, etc)

• signature of parent or legal guardian
