ENTRANCE
AGE AND OTHER STATE REQUIREMENTS
Kindergarten students must be five (5) on or before
September 1st. First year students must be six (6) on
or before September 1st. A certified birth certificate
must be presented at registration as proof of age. A
bill passed by the Georgia State legislature and signed
into law in 1992 requires that each student must have a
social security number for the school year for school
purposes.
IMMUNIZATION
Georgia law
requires that every child enrolling in a Georgia school
have an adequate certificate of immunization (Form
3231). This certificate must be signed by a Georgia
physician licensed under the laws of the state of
Georgia or by a qualified employee of the
Health Department of Human Resources and marked
"Complete for School Attendance."
EAR,
EYE and DENTAL EXAMINATIONS
A Georgia Ear, Eye
and Dental Form 3300 is required for all students.
All 3 exams must be completed for enrollment. These
forms may be obtained through a private physician or one
of the state's health departments. |
