Permission Slips

Before any student is allowed to attend any off-campus activity with the Walton Model UN Team, the appropriate permission slips must be competed and turned in to Mr. Nixon, Miss Juckett or Mr. Orff by a specified time.

Blanket Permission Slip
This is a one-time form that will be good for the entire school year.

Overnight Permission Slip
A new form is needed for every off-campus over-night conference (William and Mary and UGA)

Medical Release Form
This is a one-time form.  It needs to be notarized by a public notary before being turned in.  There is no public notary service at school.
