Registration Requirements

The following is a brief summary of documentation a student will need to be officially and finally enrolled in the Cobb County School District.  A complete explanation of enrollment requirements and documentation may be found in District Administrative Rule JF (Admissions/Withdrawals).  Items noted with a (*) are required.

r Appointments - (770) 578-3228

Appointments are required for registration to ensure that the appropriate counselor will be available.  The parent or guardian must be present to register a student who is under the age of eighteen.  We prefer that the student be present during the registration.  During the school year, a withdrawal form from the previous school attended must be presented before a student registers.

r Residence Requirement (2 proofs of residence are required.) *

Two documents proving residency (for example: home ownership title; lease or rental agreement; current utility monthly statement; current Georgia driver’s license or Georgia identification card; current bank, credit union, or other financial institution documentation; mail delivered by the United States Postal Service; employer; a Cobb County property tax statement; voter registration documentation from Cobb County; a current motor vehicle registration (tax receipt); statement of legal residence (Form JF-2)

r Proof of Immunization and Dental Screening Certificate *

A Certificate of Immunization (Georgia Form 3231) or Religious waivers (Form JLCB-2).  Please note that the immunization information must be on the GA Form 3231, so if you have immunization records from a different state you must have them transferred over to the GA Form 3231.  Students new to Georgia schools or students coming from private school must submit the Dental Screening Certificate (Georgia Form 3300).  Certification may be obtained from:

  1. A private physician and/or dentist OR

  2. East Cobb Community Health

r Transcript of Record and Grading Scale from Previous School *

All academic, disciplinary, and special education records from a student’s previous school(s).  Any student who is enrolling in Cobb County School District for the first time in grades seven or higher must provide a certified copy of his/her scholastic and discipline record from the school previously attended.  In lieu of providing a certified copy of a discipline record and transcript, the student may be admitted on a conditional basis by submitting a properly executed release authorizing the District to obtain such information from the school previously attended.  In order to place students in the appropriate courses, it is essential that a copy of the transcript be brought to registration.  A recent report card or withdrawal form is helpful, but does not ensure placement in required courses.  The counselor cannot be responsible for accurate placement if no school records are present.  A grading scale from the previous school should be included in the school records.    

r Address and Phone Number of Previous School *

In order to promptly request records from the students previous school, it is essential that the address and phone number of the previous school be presented at the time of registration.

r Social Security Number *

An official copy of the student’s Social Security card/number. (A parent or guardian who objects to the incorporation of the social security number into school records of a child may have the requirement waived by signing a statement objecting to the requirement).

r Special Education Services

All students requiring Special Education Services need to identify themselves when making a registration appointment.  Please bring the parent copy of the most recent individualized education plan and any other pertinent paperwork.

r ESOL Services

All student requiring ESOL services need to make an appointment with the International Welcome Center at 770-819-2383.

The School May:

  • Request proof of custody or custody documentation

  • Suggest that the adult with whom the student lives obtain legal guardianship
