Mary Lindsay, BSN, RN

School Nurse

Clinic Hours: 7:15-2:15

Contact me via email at Mary.Lindsay@cobbk12.org or via clinic phone by calling the school’s main line at 770-509-5162 and asking to be transferred to the clinic.

We support our students by providing immediate assessment and first aid for illnesses and injuries that occur at school. Minor injuries are treated, and parents are contacted when the injury or illness is of a more serious nature so that they may pick up their student. All medications must be provided by the parent or legal guardian of the student. Below are a couple of reminders of the CCSD policies and procedures for the clinic. Together, we can help ensure your child stays safe, healthy, and ready to learn.

IS MY CHILD WELL ENOUGH TO GO TO SCHOOL?

https://media.cobbk12.org/media/WWWCobb/medialib/form_jgc-2020.832ecd94931.pdf outlines policies and guidelines set by the CCSD. *It is critical that your child is symptom-free without medication for 24 hours before sending them back to school.

FORMS AND CARE PLANS

Please visit this link to find the appropriate forms and action plans you need:

https://www.cobbk12.org/medical-forms-and-care-plans

All medications require an Authorization to Give Medication Form to be completed.

*If your student has multiple medications, each medication will require its own Authorization to

Give Medication form.

This authorization form (JGCD-2) must be signed by the parent AND the physician for ALL prescription medications.

For students with asthma, diabetes, seizures, allergies, chronic illness, or sickle cell disease, the appropriate care plan/action plan must also be completed in addition to the authorization form (i.e. if dropping off EpiPen and Benadryl for peanut allergy, please ensure you are also bringing allergy action plan).

Other medication forms you might need:

All medications must be brought into the clinic by the parent/legal guardian of the student. To help keep your child safe and healthy, I need to ensure all necessary paperwork is completed before any medication can be given at school. Clinic hours are M-F from 7:15-2:15. The best time for parents to bring in medications to the clinic is from 7:30-10:00.

All medications must be in their original labeled container. Medications should not be sent in bags or unlabeled containers.

Please check the expiration date of the medication.

Expired medications will not be accepted.

Over-the-Counter (OTC) Medication Requirements

  • Per Cobb County Policy, a general supply/stock of over-the-counter medications is not kept in the clinic. If you feel your child may need any over-the-counter medications throughout the school year, parents will need to fill out an Authorization to Give Medication form and bring the medication, in its original container, to the clinic.
  • The Authorization to Give Medication form only requires a parent/guardian signature for Over-the-Counter medications. *All prescription medications require the doctor’s signature in addition to the parent/guardian signature.

Please note: At school we try to meet the immediate medical needs of your child and then allow you, the parent, to decide the next course of action. If your child’s symptoms continue or you have any concerns regarding their condition, we urge you to consult with your child’s doctor. Don’t hesitate to contact me if you have any questions :)
