HTMS Clinic Information

The direct number to the HTMS Clinic is 770-578-7235. Please use this number when contacting Nurse Cindy Dockery in the clinic. 

The clinic is open daily from 9:00 am until 4:30 pm. A registered nurse operates the clinic. Please refer to the student handbook for appropriate information concerning medications at school.

Two forms are necessary depending on the medication to be issued at school:

If your child needs to take medication on an ongoing basis use the Authorization to Give Medication at School form (Form_JGCD-2.docx.) This includes all prescription medicine, cough and cold medicine (except lozenges), antihistamines, and over-the-counter medication not listed below.

Middle school students may carry Tylenol, Advil, and their generic forms, antacid, and throat lozenges. The Over-The-Counter Medication form (Form_JGCD-7.docx) must be on file in the clinic and signed by parent, student, and nurse.
