Scoliosis Screening

•Scoliosis screening is required for all 6th and 8th grade students.

•The students have seen the scoliosis video about what to expect during the screening.

•Screening starts Thursday, March3, 2022 during connections by trained school staff.

•Parents that do not want their student screened at school, must complete the opt-out form.  Screening opt-out letter,  Spanish letter. You can also submit form 4400 (Spanish) from your doctor if your student was screened by their doctor.

•Letters and/or forms must be received by March 1, 2022 in order to opt-out of the screening.
