PLEASE NOTE: PER CCSD POLICY, ALL MEDICATION THAT IS KEPT IN THE CLINIC MUST BE DROPPED OFF DIRECTLY BY PARENT/GAURDIAN WITH THE SCHOOL NURSE AT THE SCHOOL. PLEASE DO NOT SEND IN PRESCRIPTION MEDICATION WITH YOUR CHILD, AS NURSE VANESSA WILL NOT BE ABLE TO ACCEPT IT FROM THEM. I know this is a lot of information, but please read through it carefully. I like to provide parents with all the pertinent information upfront, before you come, so there aren’t any surprises and wasted trips back-and-forth to the clinic. Thank you!
DROPPING OFF MEDICATION AT SCHOOL
When dropping off medication for your child at school, please make sure the following requirements are met:
- PLEASE DO NOT SEND YOUR STUDENT IN WITH MEDICATION – All medications should be dropped off directly to the school nurse by the parent/guardian – the front office staff is not allowed to accept medication, and the nurse cannot accept medication directly from a student.
- All medications should be in their original container (i.e. no loose pills in a Ziplock baggie)
- The Authorization to Give Medication form (form JGCD-2) must be signed by the parent/guardian AND the provider for ALL Rx medication. (Over-the-Counter medications do not require a provider’s signature)
- The Rx label instructions should be consistent with the instructions written by the provider on the JGCD-2 form.
- All Rx medication must have the following clearly and legibly printed on the label:
- First and Last Name of Student
- Name of Medication
- Dose
- Pill Description -- the pill description on the label should match the pills inside the bottle
- Time(s) to Administer. [NOTE: please make sure that the time(s) you want your child to receive their medication at school is consistent with the Rx label and the administration instructions provided on the JGCD-2 form].
- Please check the expiration date, as the school nurse cannot accept expired medication
- Please bring in no more than a 30-day supply of daily medication
We know this is a lot of information, but Nurse Vanessa likes to provide parents with all of it upfront, so there are no surprises and wasted trips back-and-forth to the clinic. Please email Nurse Vanessa at vanessa.moulthrop@cobbk12.org to schedule a time to drop off the medication. The best times to come to the clinic tend to be between 9:15 –11:30am and after 2:30pm.
