All members of the Class of 2026 are required to complete the Graduation Information Form. When completing the form, seniors must complete the following: 

Name Verification – If the student believes his/her name is listed incorrectly, the student is able to request changes. If not already on file, a birth certificate may be requested. 

Pronunciation – If the student’s name has a unique pronunciation, the student is able to list the correct pronunciation. 

Graduation Plans – The student indicates whether he or she will complete graduation requirements in December or May.

Commencement Plans – The student selects whether he or she will participate in the commencement ceremony. 

Contact Information – Student enters a personal cell phone number and personal email address, not the CCSD email. 

To access the Graduation Information Form, students will access the Student Portal at studentportal.cobbk12.org and sign in using their student email address and Office 365 username and password.
