*If an athlete has previously tested positive for COVID-19, It is recommended not to attend mass physicals, as we encourage the athlete to be seen by their primary care physician.*
All athletes will be screened before entering and are asked to wear a mask throughout. *All proceeds benefit Spartan Sports Medicine* Please download the athletics participation packet from link attached, or pick up a copy in the main office and bring it with you. Parents/guardians please complete history section of packet and make sure all signatures are complete prior to arriving to physical night. No physical can be provided without all parental signatures and consents being submitted in advance. Parent/guardian are recommended to accompany the athlete for physicals

We will hold our annual Physical Night this Thursday in the Gwyn-McClure Gym. This is a great way to fulfill all Physical/Waiver requirements for the 2021-22 school year, including summer training. We ask that you do your best to adhere to the schedule on the attachment, but you can jump in with another group if absolutely needed. Please make sure the physical paperwork is complete prior to attending, and you must pay through the My Payments Plus link in advance. Contact information can be found on the attachment.
