COST:  $20.00.  Students may pay online through www.mypaymentsplus.com

WHO CAN COME?:  ALL CHS Athletes Accompanied by their Parent/Guardians.  Parent/Guardian is required.   No students will be allowed to get a Physical Exam without their Parent/Guardian present.

Please download and print the athletics participation packet and bring with you!  Parents OR guardians need to complete the history section of the forms and make sure all signatures are complete prior to arriving.  Again, Parent/Guardian must accompany the athlete for physicals.  If you have questions, contact Bryan.
